The HL vs SL Difference
The translation scale above is primarily for Higher Level (HL) subjects. HL subjects are notoriously difficult (comparable to second-year college courses). Admissions officers will often look at a 5 in HL Physics and mentally treat it with the same respect as an 'A' in an AP Physics class, knowing the IB curriculum is vastly deeper.Standard Level (SL) subjects are less rigorous. A '5' in an SL class will almost universally be viewed as a 'B'.
The Strategy: Do not try to manually convert your IB scores into a 4.0 GPA on the Common App. Leave it exactly as it is (out of 45, or individual 1-7 scores). The admissions officers at elite universities are highly trained in reading IB transcripts, and if you try to Americanize it yourself, you will likely undersell the rigor of your accomplishments.
